# SolaceMed — Persisted application governance records

**SOLACEMED-GOVERNED-RUNS-001 · Observed application behavior, bounded**

Published 11 October 2026. This is a retrospective publication of 39 participant-supplied application records dated 22 April–11 September 2026, involving demo clinical encounters. It is not a prospectively frozen examination or independent attestation.

## Demonstrated application behavior

The export records state-dependent local preflight/model gating and later external Harmonic-controlled response presentation. It preserves raw model output, governed output, decisions, reasons, state flags, timestamps, and runtime metadata.

| Trace | Recorded transition | Earned observation |
|---|---|---|
| Respiratory encounter `6914076e-787c-4e45-be8d-ca7c5490311a` | 18 May 19:01 UTC: discharge request conditional; 19:09 UTC: new worsening respiratory findings, assessment escalated; 19:14 UTC: escalation request remains escalated | Earlier local implementation recomputed disposition after represented state change; prior conditional progression was not silently retained. Intent also changes, so this is an observational workflow sequence, not a controlled same-action pair. |
| Ankle encounter `fdef00b6-b144-4c20-a77a-1f310c83fddf` | 8 June 10:18 UTC: assessment insufficient; 10:19 and 10:20 UTC: added structured vitals, assessment conditional | Earlier local implementation distinguished incomplete state from review-required progression after evidence supplementation. |
| Run `d5507cff-9e06-410b-9432-b8ec5ff20823` | Model generated assessment; Harmonic `REFUSED/refuse`; governed output says presentation blocked | Recorded external Harmonic refusal controlled the application response-release disposition; determination and receipt IDs are recorded. |
| September 8 14:50 UTC run | Expected runtime 4.0.0; received 4.1.0; presentation blocked | Recorded contract failure caused fail-closed presentation. |

## Lineage and boundaries

Earlier records identify `gateway=local` preflight gates or `gateway=vercel` model processing with local governance. An OIM structural `PERMIT` in the May trace is not a native Harmonic execution permission; final discharge disposition remained conditional. Only the two later records explicitly identify `gateway=harmonic`, one contract failure and one refusal. These are evidence of their respective application implementations; earlier behavior is not retroactively attributed to external Harmonic.

The recorded consequence boundary is response presentation and model invocation. The SQL export does not independently establish browser delivery, actual discharge, medication administration, an external clinical-system effect, or a complete independent authority-change chronology. No native Harmonic `PERMITTED` release positive control appears in this export. Database timestamps and identifiers are preserved as supplied; live origin and receipt contents have not been independently verified.

## Preservation

`SOURCE_GOVERNED_RUNS_SQL.txt` preserves the uploaded source bytes unchanged. It is archival text and must not be executed as a migration. `GOVERNED_RUNS.json` is a mechanically decoded, chronologically sorted projection. `SUMMARY.json` records counts and scope. Hashes establish file integrity, not external truth. Findings do not upgrade any earlier examination or transfer automatically to later implementations.
